1. Agreement to these terms
These terms govern your use of the Subjective Health website. By using the site, including the intake form on our /start page, you agree to them. If you do not agree, please do not use the site.
2. Information only, not medical advice
This site is for general information about Subjective Health. It does not provide medical advice, diagnosis, or treatment, and it is not a substitute for the advice of a qualified clinician.
Using this site, or submitting the intake form, does not create a clinician-patient relationship and is not a medical consultation. Nothing is prescribed from a form. If you have a medical concern, speak with a clinician. In an emergency, call 911.
3. Eligibility
You must be at least 18 years old to use this site. By using it, you confirm that you are 18 or older.
4. Acceptable use
Use the site only for lawful, personal purposes. Do not attempt to disrupt or gain unauthorized access to the site, scrape it at scale, submit false or harmful information, or misuse the intake form. We may limit or end access that creates risk to the site or to others.
5. Intellectual property
The site, including its text, design, graphics, and other content, belongs to Subjective Health or its licensors and is protected by intellectual-property laws. You may view and share the content for your own personal, non-commercial use. You may not copy, republish, resell, or create derivative works from it without our permission, except where the law allows.
6. Third-party links
The site may link to websites or services we do not control. We provide those links for convenience and are not responsible for their content, accuracy, or practices. Visiting a third-party site is subject to that site's own terms and privacy policy.
7. Disclaimers
The site is provided on an as-is and as-available basis. We work to keep it accurate and available, but we do not warrant that it will be uninterrupted, error-free, or fit for a particular purpose. To the fullest extent permitted by law, we disclaim all warranties, express or implied. Content on the site is for general information and is not medical advice.
8. Limitation of liability
To the extent permitted by law, Subjective Health is not liable for indirect, incidental, special, or consequential damages arising from your use of, or inability to use, the site. Nothing in these terms limits liability that cannot be limited by law.
9. Indemnification
You agree to indemnify and hold Subjective Health harmless from claims arising out of your misuse of the site or your violation of these terms, to the extent permitted by law.
10. Dispute resolution; binding arbitration; class-action waiver
Please read this section carefully — it affects how disputes between you and Subjective Health are resolved and limits how you can bring claims.
Informal resolution first. If you have a dispute, email privacy@subjective.health with a description and what you want. We will try in good faith to resolve it, and you agree to try informal resolution for at least 60 days before starting arbitration.
Binding arbitration. If we cannot resolve it informally, you and Subjective Health agree that any dispute arising out of or relating to these terms or the service will be resolved by final and binding individual arbitration, administered by a recognized arbitration provider (such as the AAA) under its consumer rules, rather than in court. The Federal Arbitration Act governs this agreement to arbitrate.
Class-action and jury-trial waiver. You and Subjective Health agree that each may bring claims against the other only in an individual capacity, and not as a plaintiff or class member in any purported class, collective, consolidated, or representative proceeding. The arbitrator may not consolidate more than one person’s claims or preside over any class or representative proceeding. You and Subjective Health also waive any right to a jury trial.
Exceptions. Either party may bring an individual claim in small-claims court, and either party may seek injunctive or equitable relief in court to protect intellectual property or stop unauthorized use. Nothing here waives any right that cannot be waived under applicable law, and this section does not limit the authority of a government agency, such as the FTC or a state attorney general, to enforce the law.
Your right to opt out. You can opt out of this arbitration and class-waiver agreement within 30 days of first accepting these terms by emailing privacy@subjective.health with your name and a statement that you opt out of arbitration. Opting out does not affect any other part of these terms, and we will not penalize you for it.
If any part of this section is found unenforceable, the rest still applies; if the class-action waiver is found unenforceable as to a particular claim, that claim proceeds in court while the remaining disputes are still arbitrated.
11. Governing law
These terms are governed by the laws of the State of Delaware, without regard to its conflict-of-laws rules. Any dispute relating to the site or these terms will be handled in accordance with that law.
12. Changes to these terms
We may update these terms as the site grows and as the law changes. When we do, we will revise the date at the top of this page. Continuing to use the site after a change means you accept the updated terms.

Membership terms
These terms apply when you buy a Subjective Health membership. They cover automatic renewal, billing, and cancellation. Version 2026-07-v1.
M1. Your membership
A Subjective Health membership is a paid, recurring subscription that gives you access to our platform and care team on one account. You choose a billing rhythm at checkout — monthly, quarterly, or annual — and the price for that rhythm is shown to you before you pay.
Paying for a membership does not by itself create a prescription or a course of treatment. Any clinical plan is built by a licensed clinician after they review your intake. If a program is not clinically appropriate for you, you are entitled to a full refund (see our Refund Policy).
M2. Automatic renewal
Your membership renews automatically at the end of each billing period at the same price and cadence you selected, and your payment method is charged, until you cancel. By starting checkout you agree to this automatic renewal. You give a separate, affirmative consent to automatic renewal on our join page before payment, and again on the secure Stripe checkout page.
Before each renewal, we send you a reminder by email that states your plan, the renewal amount, the renewal date, and a link to cancel. You do not need to do anything to continue; you only need to act if you want to cancel or change your plan.
M3. Billing
Payments are processed by Stripe. Your card statement will show the descriptor SUBJECTIVE HLTH. Prices are in US dollars and are charged at the start of each billing period. Applicable taxes, if any, are shown at checkout.
If a renewal payment fails, we may retry it and will let you know. Your access may continue during a short grace period; if payment cannot be completed, your membership may be paused or canceled.
M4. How to cancel
You can cancel anytime, in two clicks, from the Manage membership page at subjective.health/manage. You do not have to call anyone or send a letter. Enter the email you used at checkout, open the secure billing portal, and cancel there.
When you cancel, your membership stays active until the end of the billing period you have already paid for, and it does not renew again. Refunds, where they apply, are governed by our Refund Policy.
M5. Plan changes
You can switch plans from the billing portal. A member who wants to step down rather than cancel may move to a lower-cost maintenance plan and keep access. Changes take effect according to the terms shown in the portal at the time you make them.
M6. Refunds
Our Refund Policy is a part of these membership terms and is available at subjective.health/refund-policy. It describes the 30-day full-refund window, pro-rata refunds after that window, and the clinical-appropriateness refund.
M7. Changes to membership terms
We may update these membership terms and our prices. If we change the price of your current membership, we will tell you in advance and give you a chance to cancel before the change takes effect. Continuing your membership after a change means you accept it.
